The Brain-Heart Connection: Breathing, HRV and Stress
Learn how breathing, heart-rate variability and the autonomic nervous system are connected, with gentle practices that may support stress regulation.

The brain, heart, breathing and nervous system continuously exchange information. This communication helps the body respond to changes in physical activity, stress, emotion, rest and the surrounding environment.
You may sometimes hear this relationship described as “brain-heart coherence” or “brain-heart entrainment.” These terms are commonly used in wellness and biofeedback settings, but they should not be understood as a medical diagnosis or a guaranteed state of emotional or physical health.
A more practical way to explore the brain-heart connection is through the autonomic nervous system, breathing and heart-rate variability.
How the Brain and Heart Communicate
The brain and heart communicate through several interconnected systems, including:
The autonomic nervous system
The vagus nerve
Hormonal and stress-response systems
Changes in breathing
Blood-pressure regulation
Sensory information from within the body
The autonomic nervous system helps regulate involuntary functions such as heart rate, breathing, digestion, blood pressure and aspects of the stress response.
It is commonly described as having two principal branches:
The sympathetic nervous system, which helps mobilize energy and respond to demands
The parasympathetic nervous system, which supports functions associated with rest, digestion and recovery
Both branches are necessary. Regulation does not mean remaining in a parasympathetic or completely calm state. It involves having sufficient flexibility to respond to changing circumstances and recover after activation.
The Role of the Vagus Nerve
The vagus nerve is an important communication pathway connecting the brain with several internal organs, including the heart, lungs and digestive system.
It carries information in both directions. Signals travel from the brain toward the body, while substantial sensory information also travels from the body toward the brain.
The vagus nerve contributes to functions such as:
Heart-rate regulation
Breathing
Digestion
Reflexes related to swallowing and coughing
Communication between the brain and internal organs
Vagal activity is complex and cannot be accurately assessed simply by noticing whether you feel calm or by checking one reading on a consumer device.
What Is Heart-Rate Variability?
Heart-rate variability, or HRV, refers to variation in the amount of time between consecutive heartbeats.
A healthy heart does not beat like a perfectly regular metronome. The interval between beats changes in response to breathing, movement, stress, rest and other physiological demands.
HRV can provide information about cardiac and autonomic regulation, but it must be interpreted carefully. It is affected by many factors, including:
Age
Physical fitness
Breathing rate
Sleep
Medication
Alcohol
Illness
Time of day
Physical position
Measurement method
Emotional and physical stress
A single HRV number is not a diagnosis, a measure of emotional health or a simple score of how well your nervous system is functioning.
Higher is not always automatically better, and comparisons between different people may not be meaningful. Trends measured consistently may provide more useful information than isolated readings.
Why Breathing Influences Heart Rate
Breathing and heart rate are naturally connected.
For many people, heart rate increases slightly during inhalation and decreases during exhalation. This pattern is called respiratory sinus arrhythmia.
Slower breathing can influence the timing and size of these variations. Research suggests that voluntary slow breathing can increase certain HRV measurements during practice and may support aspects of autonomic regulation.
This does not mean that slow breathing synchronizes the brain and heart into a perfect state or treats every stress-related concern. It is better understood as one possible self-regulation practice.
A Gentle Paced-Breathing Practice
Try this exercise only if focusing on your breathing feels comfortable.
Sit in a position that feels supported.
Keep your eyes open or closed, depending on your preference.
Notice your natural breathing without changing it immediately.
Allow your breathing to become slightly slower and easier.
Let the exhale be comfortable rather than forceful.
Continue for one or two minutes.
Return to your normal breathing and notice how you feel.
The intention is not to take the deepest breath possible. Breathing too deeply or quickly can cause light-headedness, tingling or increased anxiety.
If you feel dizzy, short of breath, panicked or uncomfortable, return to your natural breathing and stop the exercise.
People with respiratory, cardiovascular or other relevant medical conditions should consult an appropriate healthcare professional before beginning a structured breathing program.
What If Focusing on Breathing Increases Anxiety?
Breath-focused exercises are not calming for everyone.
Some people become more anxious when asked to monitor or control their breathing. This may be particularly relevant for people who experience panic, health anxiety, respiratory concerns or certain trauma-related responses.
If breathing feels uncomfortable, you can use an external anchor instead.
Try noticing:
The support of the chair beneath you
The feeling of your feet against the floor
Several objects in the room
Sounds near you and farther away
The temperature of the air
An object with a distinctive texture
Regulation strategies should be adapted to the individual rather than treated as universal instructions.
Mindfulness and the Brain-Heart Connection
Mindfulness involves intentionally observing present experience with less immediate judgment or reaction.
During mindfulness practice, you might notice:
Breathing
Physical sensations
Thoughts
Emotions
Sounds
Changes in tension or energy
The impulse to move, speak or withdraw
Mindfulness does not require an empty mind or a particular physiological state. The purpose is to develop awareness of what is occurring and create more choice in how you respond.
A brief practice might involve noticing three natural breaths, three physical sensations and three sounds. If internal sensations feel overwhelming, you can direct more attention toward the external environment.
What Is HRV Biofeedback?
HRV biofeedback uses a sensor and visual or auditory feedback to show changes in heart rhythm, often while a person practises paced breathing.
The objective is generally to help the person observe how breathing and attention influence physiological patterns in real time.
Research suggests that HRV biofeedback may help reduce self-reported stress or anxiety for some populations. However, study methods and results vary, and more high-quality research continues to be needed.
HRV biofeedback should not be presented as a cure for anxiety, trauma, cardiovascular illness or another medical or psychological condition.
When biofeedback is being used for a clinical concern, guidance from an appropriately trained professional may be helpful.
Consumer Devices and HRV Scores
Watches, rings and other consumer devices increasingly provide HRV estimates.
These measurements can be influenced by:
Device quality
Sensor placement
Movement
Recording duration
Time of measurement
The device’s proprietary calculation method
HRV values from different devices may not be directly comparable.
Constantly checking physiological data can also increase anxiety for some people. If tracking causes worry, compulsive checking or attempts to control every reading, taking a break may be more supportive than gathering additional data.
Consumer HRV measurements should not replace medical assessment or professional advice.
Potential Benefits of Breathing and Biofeedback Practices
Slow, comfortable breathing and HRV biofeedback may support:
Awareness of stress responses
Recognition of changes in activation
Development of a regular self-regulation practice
Greater familiarity with physical signs of stress
A structured pause during emotionally demanding moments
Reduced self-reported stress or anxiety for some people
These are possible benefits, not guaranteed outcomes.
The evidence does not support promising that brain-heart practices will improve immunity, prevent cardiovascular disease, lower blood pressure, cure anxiety or produce emotional transformation.
Integrating Body Awareness Into Therapy
In therapy, awareness of breathing and heart rate is not used to determine whether you are succeeding or failing at regulation.
Instead, physical signals may provide information about how you are responding in the moment.
For example, you may notice:
Faster breathing during a difficult conversation
Tightness in your chest when discussing a boundary
An urge to withdraw when you feel criticized
A sense of settling when you feel understood
Numbness or disconnection when emotions become intense
We can explore these experiences with curiosity and without forcing them to change.
My approach may integrate somatic awareness, mindfulness, Internal Family Systems-informed therapy, EMDR and other evidence-informed methods according to your needs.
Regulation Is Not the Same as Constant Calm
A regulated nervous system does not remain calm at all times.
Anger, fear, grief, excitement and activation are normal responses. The goal is not to eliminate them or maintain a perfect HRV score.
Regulation may involve:
Recognizing what you are experiencing
Understanding what an emotion is communicating
Remaining present with a manageable level of activation
Taking protective action when necessary
Asking for support
Returning toward your usual state after stress
Responding with greater flexibility rather than automatically
No breathing technique can remove the need to address unsafe circumstances, relationship problems, medical concerns or other sources of distress.
When to Seek Professional Support
Consider speaking with a qualified healthcare professional if you experience:
Chest pain
Fainting
Significant shortness of breath
A new or persistent irregular heartbeat
Concerning changes in heart rate
Physical symptoms that interfere with daily functioning
These symptoms should not be assumed to result from anxiety or nervous-system dysregulation.
